          For the formative assessment assignment, I created a "impossible to fail quiz" using a google form.  This is type of assessment is also referred to as a "choose your own adventure" assessment or activity.  In order to advance to the next portion of the google form, a student must answer the question correctly.  This can be done through multiple choice type answers, or fill in the blank answers using data validation.  More information on this topic can be found here.

          I like this type of assessment because it is about so much more than testing a student's ability to answer a question correctly.  Students are given the opportunity to retry questions that they miss.  There are also videos or info graphics provided to students before they attempt a question and additional help is provided if the student misses the question before attempting the question again.  Many teachers are providing this type of remediation with "retests" but that remediation is built directly into this assessment.

          This would be a great tool for a formative rather than a summative assessment.  This activity is effective over a traditional worksheet because students have accountability for answering questions correctly.  They have to take the time to show their work when calculating their answer.

          Technology also makes grading this assessment much easier.  This google form does not allow students to advance until their answer is correct.  However, any google form could be easily graded through a google forms quiz, flubaroo, or through collecting answers in a google sheet and writing a program that checks the students' answers.
